Virtual Physical Therapy Evaluation

Service Description

A one-on-one evaluation of your symptoms with a Doctor of Physical Therapy performed virtually. This consists of an interview of questions regarding your symptoms, a movement exam, development of a plan of care specific to your goals, and initial treatment. No referral is necessary to begin physical therapy and this service can be reimbursed by insurance.

Cancelation Policy

Appointments which are missed or cancelled less than 24 hours in advance will result in a cancellation fee. A cancellation fee of 50% of the visit cost will be charged for the first visit missed or cancelled appointment, and the full amount of the visit will be charged for any other visit following. Cancellations can be done by phone, email, or text and your visit will be rescheduled as soon as possible. Special cancellation situations will be considered on a case by case basis.
